• SWIFT program receives 2023 Outstanding Rural Health Program Award

    On the evening of Monday, Oct. 23, 2023, at the Maryland Rural Health Association (MRHA) Annual Conference, the Salisbury-Wicomico Integrated Firstcare Team (SWIFT) received the 2023 Outstanding Rural Health Program Award from Jonathan Dayton, MRHA Executive Director.
     
    This SWIFT program, a collaboration between TidalHealth’s Community Wellness team and the Salisbury Fire Department (S.F.D.), is focused on connecting high utilizers of 911 and emergency department services with primary care and specialty services.
     
    The program further operates the Minor Definitive Care Now (MDCN) program, where a TidalHealth nurse practitioner and an S.F.D. paramedic respond to low acuity 911 calls to assess and treat patients on scene to prevent unnecessary emergency department visits. The MDCN program is the only program functioning in the State of Maryland and is one of only 25 programs nationally providing this innovative and cutting-edge service.
