Blood Pressure Checks and Diabetes Risk Assessments

The University of Maryland Shore Community Outreach Team serves residents of Dorchester County. We will be at Bradford House, 701 Race Street, Cambridge on 10/15/25 from 2-3:30 pm
Our mission is to improve the health of the community by bringing health care access and support services to residents in their home and community settings.
Combining expertise in nursing and social work, Shore Community Outreach Team offers many services for individuals and families, including:
Home safety inspections
Blood pressure checks and health screenings
Case management for those with chronic illnesses (COPD, heart disease, diabetes, etc.)
Wllness activities
Arrangement of telehealth consults with physicians and other clinicians
Social visits for the homebound
Connections to community resources
